FIRST TIME APPLICANTS MUST APPEAR IN PERSON TO BE FINGERPRINTED AND PHOTOGRAPHED.

 

1-APPLICATION FORM:
Answer all questions truthfully and completely.
Notarize applicant's signature.
Any individual who assist the applicant in completing the application must complete the "Interpreter Information" section of the driver
application.   

2-REFERENCES:
Three (3) persons not related to the applicant by blood or marriage must complete a form.  Each form must be notarized.  (Reference forms are not required for renewal applicants). 

3-TWO (2) FORMS OF IDENTIFICATION:
One form of identification must be a driver license, class E or better (or equivalent form driver's home state).  If you drive a vehicle with a seating capacity of 15 or more passengers you are required by NYS DMV and WCTLC to have a commercial driver license.  The second form of identification must be from the list supplied in the "Driver Identification" section of the driver application. 

4-SOCIAL SECURITY CARD:
You must submit a valid, unaltered, original Social Security card.  An original work authorization form must also be supplied if the following is stated on the Social Security card: "Valid For Work Only with  DHS [Or INS] Authorization".  WCTLC staff will make a copy of each and attach the copies to the application.

This is a one-time requirement for all renewal applicants whose permits expire between January 1, 2006 and December 31, 2006.  As a result, driver renewal applicants will be required to submit their applications in person.  APPLICATIONS WILL NOT BE ACCEPTED BY MAIL DURING THIS TIME PERIOD.

5-PAYMENT:
Provide two separate money orders or company checks:

  1. FINGERPRINT FEE - $80, MAKE PAYABLE TO "Westchester County".   (Fingerprint fee not required for renewal applicants).

  2. REGISTRATION FEE & DRUG TESTING FEE - $125, for first time        applicants and $115 for Renewal Applicants, make payable to "WCTLC".

  3. LATE FEE (Renewal applicants only) - Renewal applicants that submit their applications late are required to pay an additional $25, make payable to "WCTLC".

6-SELF-ADDRESSED, STAMPED ENVELOPE:
Supply a standard size envelope with proper first class postage.  Print name and address in the center of the envelope.  If your application is approved this envelope will be used to mail you your permit.

7-DEFENSIVE DRIVING COURSE:
Supply a copy of your Certificate of Completion from a NYS  approved, six (6) hour defensive driving course showing that you completed the course within eighteen (18) months for new applicants and three (3) years for renewal applicants.

8-OUTSTANDING FEES AND/OR FINES:
All outstanding fees and/or fines owed to WCTLC must be paid before an application will be approved.

9-DRUG TESTING:
You will be given further information as to what procedures to follow for drug testing when you submit your completed application.
FAILURE TO PASS OR TAKE THE DRUG TEST WITHIN THE
REQUIRED TIME FRAME WILL CAUSE YOU TO BE DENIED A WCTLC
PERMIT.
