Program Highlights:
The
Infant-Child Health Assessment Program identifies children at risk for developmental
delays and provides the following services:
- home visits by a public health nurse
- health education
- developmental screening tests for monitoring
child's development
- referrals to other community agencies as
needed
Ages:
- Infants and Toddlers under age 3
Eligibility:
Eligibility is based on evaluation results
in accordance with New York State Department of Health regulations. Evaluations are
available at no cost to the family. If it is determined that your infant/toddler is
eligible for services, your family will meet with a public health nurse from the
Department of Health in your home.
Cost:
Infants and toddlers at risk for developmental delay will
receive home visits by a public health nurse at no cost to the family.
