How to make a complaint about a Food Service Establishment

Please report to the Westchester County Department of Health as soon as possible if you see or experience:

  • Inappropriate food handing practices by food workers
  • Unhealthy conditions

Each complaint receives an appropriate response from public health sanitarians.

Call 914-813-5000 to register a complaint (8:30 am - 4:30 pm).  Include the following:

  • Your name and a way to contact you.  Complaints can be made anonymously if you can supply the facilities full address.  However, this limits the Health Department’s ability to gather additional information from you, if necessary.  Also, the Health Department will not be able to report findings.
  • Name and address of the food establishment.  Inspections can only be performed at food establishments located in Westchester County.
  • What unsafe food handling practices did you observe or experience?
  • Did you or others get sick?  If so, see your physician, health care provider or emergency services. Call the Health Department immediately to report a foodborne illness at 914-813-5000.   Hours are Monday – Friday, 8:30 AM to 4:30 PM.  The Health Department maintains a 24 hour/day public health emergency phone line at 914-813-5000.
