| A student who is a resident of Westchester County, who wishes to reduce their tuition cost while attending a community college outside of Westchester, may submit a Certificate of Residence issued yearly by the Westchester County Commissioner of Finance. The certificate is used at registration at the chosen community college. |

| Company Profile
(pdf) This form should be completed for those firms wishing to be added to the
County's bid lists for materials, supplies, and equipment including service to that
equipment. |

Pre-Qualification Affidavit Form (pdf) This form must be
completed by all firms wishing to be added to the Bureau of Purchase bid
lists for materials, supplies, and equipment including service to that
equipment as a Minority Owned or Women Owned Business Enterprise (MBE/WBE).
The completed form must be mailed to the Bureau of Purchase and Supplies
at the address above. |
| W-9 Federal Form (pdf) This
completed form must be provided to the Finance Department by every new vendor/contractor
conducting business with Westchester before payment is made to such vendor. |
