Mission
Statement:

Our goal is to promote good health and to coordinate services for participants requiring primary, preventive, diagnostic, therapeutic and palliative care. Our collaborative approach contributes to peace of mind for those we serve, as well as their families and caregivers.
The program helps medically fragile seniors and individuals with chronic conditions or disabilities remain actively engaged in the world around them.
