The Transitional Management Services Program is a short term outreach program that works in conjunction with the mental health staff at the local correctional facilities to develop a community based service plan for people with serious mental illness as they re-enter the community.  

The service plan establishes a connection to specific mental health providers in the community to ensure continuity of care.  

Transitional Services staff coordinate these services with local community criminal justice agencies as appropriate.
