MR/DD Registry

The Westchester County Department of Community Mental Health is the central point of intake for residential and community-based services in the county.  The MR/DD registry is a computerized, confidential system that was established in order to identify and prioritize the needs of individuals with developmental disabilities in the county.  By providing an up-to-date listing, the registry helps to allocate resources and plan for additional or improved services for individuals with developmental disabilities and their families now and in the future.  Registration also enables the person with developmental disabilities to be considered for residential placement, day programs and support services.

Registration can be done at anytime prior to requesting services.  You do not have to be seeking services in order to register.  Registration is accomplished by completing a registration packet and a consultation with our intake specialist who will contact an individual or family after the registration packet is received.  Registration packets are available in both English and Spanish.

To obtain a registration packet call 995-5244.  Or, click below to download a registration packet.
