Enhanced
case management and rent subsidies are available for individuals who are
developmentally disabled and homeless. This project assists individuals
from the Westchester County Shelter system who have been determined to have a
developmental disability to secure appropriate, permanent housing. In
addition, enhanced case management is provided to ensure the linkage to
necessary medical, day and social services to prevent future homelessness.
