Children with Special Health Care Needs Program (CSHCN)

 

The function of CSHCN is to:
  • Help coordinate specialty care
  • Provide information about programs and resources appropriate for your child with special needs
  • Link families to community-based services such as:
    • Child Health Plus/Medicaid
    • Early Intervention (EI)
    • Women Infants and Children (WIC)
    • Developmental Disabilities Service Office (DDSO)
    • Supplemental Security Income (SSI)
    • Parent Support
  • Assist Families in obtaining the following services offered by the Physically Handicapped Children's Program (PHCP)
    • Referrals for Initial Diagnostic Evaluations
    • Orthopedic equipment
    • Hearing aids
    • Medication
    • Transportation
    • Clinic visits for specialty care

    Families must meet the income eligibility guidelines and reside in Westchester County.

    The Children with Special Health Care Needs Program is an information and referral service designed to assist the families of physically challenged or chronically ill children 0-21 years of age.
